athletes are required to submit the following by first practice:<\/p>\n\n\n\n<ol><li>Online Registration form- includes (Student-Athlete Contract, Athletic Waiver, APIAL Waiver)<\/li><li>Physical Form (signed by physicians)- Fax\/Email or Drop off in envelope with Athletes name, grade, and sport.<\/li><li>$55 payment (cash, check, money order)- Drop off in envelope with Athletes name, grade, and
